This form is to be completed and signed by a parent or legal, court-appointed guardian (if legal guardian, attach copy of court order appointing you as legal guardian). This information is to confirm identity and grant eligibility.

If your child will be under the care of a guardian, you must provide us with proof of guardianship and schedule a meeting to discuss the proper legal documents that must be submitted in order for your child to be enrolled or remain enrolled at Meadowridge School.
